Autism Awareness Enrollment
Help us serve and protect those with autism.
The Clearfield Police Department Autism Awareness Enrollment is intended to help law enforcement, EMS, Fire, and other emergency responders communicate and interact with individuals with autism. This voluntary enrollment form provides critical information for emergency personnel responding to a scene or contacting an individual.
Submitted forms will be entered into the Clearfield Police Department Records Management System and accessible by the Clearfield Dispatch Center. It is recommended that this form be updated yearly so first responders have current information when handling emergency and non-emergency situations.
Record Requests
You may request copies of police records and supports with the form below. Please bring physical record request forms to the front desk of the city building along with the record request fee. If submitted online, a fee can be paid over the phone or in person upon pick up.
Tickets & Citations
If you fail to appear, the court may issue a warrant for your arrest.
If you are issued a Clearfield Police Department Uniform Citation or Information and Summons to Appear then you, the defendant, were given notice to appear in The Clearfield Justice Court not less than (5) five nor more than (14) fourteen days after issuance of the citation, or as directed by the court. You may pay your citation in person at Clearfield City courts or online.

Chief Kelly Bennett
In 2005, he was promoted to the rank of corporal and transferred to the patrol division. Shortly thereafter, he was promoted to the rank of sergeant in the patrol division and in 2010 transferred to the detective division. In 2014, he was promoted to the rank of lieutenant and oversaw the patrol, records and code enforcement divisions. In June of 2017, he was promoted to the rank of assistant chief. Chief Bennett was appointed as Clearfield City’s Chief of Police in January of 2019. Chief Bennett has a Bachelor’s Degree in Criminal Justice and a Master’s Degree in Criminal Justice Administration
